The State of Indiana has made some changes in immunization requirements.  The changes are:

Preschool and Kindergarten – 2 Varicella vaccines or doctor documentation of disease

Grade 6 through 12:

• 1 Tdap
• 1 Meningococcal (MCV4 – Menactra)
• 2 Varicella vaccines or parent documentation of disease
